Depending on your patients’ diabetes and blood glucose goals, you may start them on insulin to help manage their glucose levels. Although you will prescribe the appropriate insulin to help your patients reach their goals, from the patients’ perspective, treating their diabetes can be challenging if insulin is added to their treatment regimen without clear instructions and follow up to make them feel at ease with the steps needed for success.

Normal functioning of the microbiota provides the host with important physiologic and immunological functions. It has been proposed that modern lifestyles and a Western dietary pattern have resulted in a loss of commensal bacteria and a reduction in the diversity of species present. This article will review the functions of the microbiota and how modern lifestyles may have contributed to changes in species available to humans.

After a five-year hiatus, Educator’s Corner is returning to the pages of Practical Diabetology. The goal of this column is to promote diabetes education and diabetes educators and to disseminate creative approaches to diabetes selfmanagement education, medical nutrition therapy and other services that improve diabetes care through education. The column will be written by experts in the field who will share innovative practices that have made an impact in the communities they serve. The articles are meant to be practical so all health-care professionals can use the tools provided to enhance their practice.

This column highlights recent clinical trial data and landmark trials to provide relevant information and links for obtaining trial data and articles to facilitate discussion with patients and other providers. The trial is identified by its acronym, its ClinicalTrials.gov Identifier (NCT Number) and its journal reference. When possible, the reference will include the study design paper and the main outcomes. If the Clinical Trial Study Group has made a slide set available, the link will be included. A summary provides primary outcome results. Selected abstracts also may be highlighted with a summary of the main points. How many times have you asked a patient in clinic for his or her glucose meter only to be told, “No one asked me to bring it”? The majority of patients with newly diagnosed T2DM who present for a consultation tell me that, despite the fact that our clinic staff specifically tells them to bring their meters and all their medications to their first visit. What about subsequent visits? Unfortunately, they say the same thing in those instances, even though I recall having had this same conversation at the last visit.
